Lizzy Care is seeking a dedicated and visionary VP of Program Development & Clinical Operations to lead our innovative interdisciplinary team and shape the future of dementia care. Reporting to our CEO, this role is critical in overseeing clinical program development, team management, and client success, ensuring that our services meet the highest standards of quality and care. You’ll recruit and train a top-tier clinical team, develop standardized assessment tools, design and implement care protocols, and build valuable partnerships with healthcare providers and stakeholders across the NY tristate area. Duties/Responsibilities:

Clinical Team Management.:

  • Recruit and train members of the interdisciplinary team
  • Lead IDT meetings and case review
  • Monitor adherence to care protocols and pathways
  • Coordinate patient and family education resources and delivery
  • Partner with medical leadership to ensure safety and compliance 

Program Development:

  • Design and implement clinical care management application specifications, including:
    • Standardized, evidence based geriatric assessment tools
    • Clinical team-based workflows to drive interdisciplinary care
    • Comprehensive care-plan design and implementation
  • Partner with product and tech teams to ensure documentation systems are in compliance with new CMMI GUIDE Model requirements for dementia care management
  • Drive systems for discrete data collection to track patient specific outcomes
  • Collaborate on network development for clinical support services

Client Success:

  • Develop strategic partnerships with healthcare providers and other key stakeholders to enhance service offerings.
  • Collaborate with internal teams to ensure alignment between partnership goals and joint venture objectives.
  • Manage agreements with JV partners, ensuring mutual benefit and compliance with health system policies.
  • Monitor and assess partnership outcomes, providing regular reports and recommendations for improvement.
  • Identify collaboration opportunities to improve patient care, streamline services, and promote growth.

Qualifications

  • Advanced clinical degree (RN, NP, MSW, MPT)
  • Minimum 5 years clinical experience post licensure
  • Minimum 2 years experience as a supervisor of clinical teams
  • Demonstrated experience with clinical principles of geriatric care and care management
  • Demonstrated experience with tech-enabled clinical program development

Location

Location: NY Tristate area (NY/NJ/CT), Hybrid (50% home office; 50% in-person with team, clients and customers as needed)

Salary

$170,000 total compensation. Including competitive total compensation and benefits package, equity option package, and performance based compensation plans.
