Understanding and document medical history, care and needs

Alongside any legal documents you will probably have some medical documents for your loved one.  Make sure that you keep an organized system of filing so that you can easily find all the information you need. A simple ring binder with tabs is a great tool for any discerning caregiver! 

Medical documents might be as simple as a list of medications or allergies or it may be a care plan created by a doctor or social worker. You should also make sure that you have a record of your loved ones insurance information including what plan they have and their membership number. Take a photocopy of their insurance card and keep it in the important documents binder. 

Make sure you know when the plan renews and set up a reminder and review any benefits your loved one is entitled to. Some plans offer supplementary benefits so aim to find out what your loved one has access to. 

Many doctors offices and medical providers keep records electronically and provide access through a portal. If you are a registered health care proxy or health care agent then you should be able to request and receive access to these files.
