How to help your loved one by staying on top of their medications

We briefly covered medications in the Discharge section of the Hospital stays part of the course but we’ll spend a bit more time on medication management here. 

It’s vital to make sure that you keep an up-to-date list of all the medications that your loved one is taking.  Your list should contain the information on what each medication is for, when the medication is taken and how much (the dosage), how long the prescription is for and any side effects to look for. 

Keep up with when your loved one will need any refills. 

Not every doctor involved in your loved ones care may be aware of all the medications your loved one is taking so make sure you share information on the full list of medications your loved one takes with each medical team. It’s also important to communicate any allergies your loved one may have. 

If your loved one is having difficulty taking medication then you can also ask the doctor or pharmacist if it is available in a liquid form. 

If you ever have any concerns or questions about the medications your loved one is taking make sure that you ask their doctor.
