We’re excited to share some big news: Lizzy Care is now part of Medicare’s new GUIDE program! This means that starting July 1, 2025 we can offer more support to people living with dementia and their families, at no cost to them, through Medicare.
What is GUIDE?
GUIDE stands for “Guiding an Improved Dementia Experience.” It’s a new program from Medicare that helps people living with dementia get the care they need to stay safe and well at home. It also helps family caregivers feel less overwhelmed.
With GUIDE, families get access to a team of dementia experts like care coordinators, nurses, and trained support staff who check in regularly and help make sure everything is on track.
Why This Matters
Dementia is hard for the person living with it and also for their loved ones. Many families don’t know where to start or how to find the right help. GUIDE changes that.
By joining the program, families can:
- Get a personalized care plan tailored to their needs
- Talk to a care navigator regularly for help and advice
- Receive 24/7 support in case something urgent comes up
- Access free respite care, so caregivers can rest and recharge
- Learn how to manage dementia symptoms like confusion or stress
- Plan ahead for future medical decisions

- A dedicated care coordinator for each family
- Regular health and wellbeing check-ins
- Help connecting to doctors, in-home support, and community resources
- Emotional support for caregivers, plus tips for managing burnout
